Your initial visit will include a routine examination and cleaning, followed by the development of a personalized treatment plan tailored to your needs.
Patient Forms
During your initial appointment, we require you to complete several forms to provide us with a comprehensive understanding of your overall health and specific issues we should address. Filling out these forms can be time-consuming, typically taking approximately 20 minutes.
